Do I need a local IIS SMTP server installed on the actual ISA server?

Answer

You need to put an IIS SMTP Server on your internal network and publish that server. After you have published the Server, configure the SMTP Service to relay messages to your internal mail server by configuring the “Smart Host” to the name or IP address of your internal Mail server.
