Can ISA block web access by scanning webpage content? Therefore if I set “sex” as a keyword, ISA would scan the webpages before serving it to the client, to check for this keyword.

ISA Server does not have this functionality built in, you will have to purchase a 3rd party add-on. Check the list of “Access Control” products on this site (http://www.isaserver.org/software/access_control.htm)
